- 1.Admission is open to Nursery Class only.
- 2.The child’s Date of Birth should fall within the dates mentioned below :Between ______ and _______
- 3.The particulars of the Child ( i.e. Name, Parent's Name and Date of Birth ) should be filled in correctly. The date of the birth filledin the form must tally with the one recorded in the Date of Birth Certificate issued by the Municipal Authority as well as the Maternity Hospital / Discharge Certificate.
- 4. Incomplete form will not be processed.
